Driving Directions

Northbound I-5:

Take I-5 northbound to the James St. exit (#164A).
Turn right on James St. and proceed east to Boren Ave.
Turn left on Boren Ave.
Turn right at Madison St. Follow Madison St. to Summit Ave.
Turn right at Summit Ave., then follow the drive around into the Madison parking garage.

Southbound I-5:

Take I-5 southbound to the James St. exit (#165).
Turn left on James St. and proceed east to Boren Ave.
Turn left on Boren Ave.
Turn right at Madison St.
Follow Madison St. to Summit Ave. Turn right at Summit Ave., then follow the drive around into the Madison parking garage.

Patients are welcome to park in the Madison Street parking garage when they visit their physician’s office. Nearby street parking is limited in this area.
