Facial Plastic Surgery

A face lift, also known as rhytidectomy, is the removal of facial wrinkles that result from aging, gravity, exposure to the sun, and the daily stresses of life. This procedure can help make you look younger and fresher, thereby enhancing your self- confidence. It is commonly performed after the age of 40, when skin starts to sag and deep creases may start to appear on cheeks and between the nose and mouth. At this time, you might also notice some folds and fat deposits around the neck, or a double chin forming.Facelift Procedure
A facelift is generally a fairly lengthy surgical procedure normally performed with a combination of local anesthesia and sedation that takes up to 2 to 5 hours. It typically begins at the temples right above the hairline, following a natural line in front of the ear, and continuing behind the earlobe to the lower scalp. If the neck requires some correction, a small incision may also be made under the chin. To improve the contour, fat may be trimmed from around the neck and chin.
Next is the tightening of the underlying muscle and membrane, and then the skin is pulled up and excess skin removed. After closing the incision, cotton dressing is used to bandage the face and head. This helps to prevent swelling and also to provide good support for 2 to 3 days. Bruising and swelling normally decrease to “socially acceptable levels” within 10 to 20 days. But remember, we all heal differently. Any significant discomfort can be reduced with some pain medication. Transgender Plastic Surgery
Transgender medicine is a growing field that assists those with a diagnosis of gender dysphoria. The recognition and treatment of this condition has become a commonly accepted area of medicine. As part of a patient’s transition, surgical options exist to assist with feelings of gender dysphoria. Some individuals become comfortable with their role and identity without surgery but for others, surgery is an essential aid to alleviate their anxiety. Gender affirming surgery can help to make patients feel more at ease in private and social settings. Studies show that subjective measures of well-being, cosmesis, and sexual function all show benefit to the individual.
A number of procedures are available to this group of patients. Male to female transition can be aided by the performance of breast augmentation and other feminizing procedures. These may include facial surgery such as rhinoplasty, forehead lift, and blepharoplasty. Liposuction, thyroid cartilage reduction, and fat transfer are other procedures that may be contemplated. Genital reassignment surgery is also an option for some. Breast augmentation procedures require only a small incision and the implantation of a breast implant.
Female to male procedures may include “top surgery” which essentially involves a mastectomy and creation of a male chest anatomy. Different procedures exist depending on the size and shape of the breasts. “Keyhole” procedures involve an incision around the edge of the nipple only and are applicable to those with small breasts. “Double incision” operations are required for those with larger breasts and often include reconstructing the nipple with a grafting procedure. Other surgeries that may be of value include liposuction, fat grating, pectoral implants, and facial procedures. Genital surgery may also be an option for select individuals.
All patients are required to have persistent, well-documented gender dysphoria. Pre-operative social transition is also a requirement. Hormone therapy prior to surgery is not required but may be recommended depending on individual circumstances.
Male to female transition procedures
Breast augmentation is commonly performed. The procedure is to place a saline or silicone breast implant either behind the existing breast tissue or under the pectoralis muscle. The incision used to place the implants is usually located under the breast in the fold and only needs to be about 1 ½ inches long. There are many different sizes of implants and the determination of which implant would be best for you will be a joint decision between you and your surgeon. The procedure takes about 1 ½ hours to complete and is performed in our outpatient surgery center. Recovery takes only a few days to return to routine daily activities and full recovery for all activities will take 4-6 weeks. Patients that are taking estrogen based hormones will likely experience some breast growth which assists in obtaining good results. On occasion, pre-expansion of the chest will be advisable prior to placing permanent implants. In this case, a device called a tissue expander will be placed on each side. Following surgery, these tissue expanders will be inflated in the office with saline using a small needle. This creates a space for the implants that will subsequently be place usually 2 months later. This process can aid in shaping the breast and allow larger implants to be placed in those patients with little or no breast development.
Liposuction is frequently employed to help create more a feminine body shape. Liposuction involves removing fat in designated areas by using a special suction device that requires very small incisions. The areas that are commonly addressed are the abdomen, hips, thighs, calves, and neck. After surgery, compression garments are worn for several weeks. The purpose of compression is to help minimize bruising and swelling and to increase comfort.
Facial feminization frequently includes a browlift procedure. The purpose of a browlift is to reposition the brows to a higher level which helps to feminize the face. Browlifts have the benefit of creating a more arched eyebrow and accentuating the entire eye area. This procedure can be performed via an incision at the hairline or endoscopically which requires on 4 small incisions behind the hairline.
Commonly referred to as an eye lift, upper eyelid surgery can help to remove excess skin and create a more feminine appearance with more of the eyelid showing. The excess skin is removed and the resulting scar is place in the upper lid crease. This procedure is commonly performed in conjunction with a browlift.
Rhinoplasty to reduce and feminize the nose is very commonly performed. Small incisions inside the nose are employed. A hump can be removed, the nose can be narrowed, and the tip can be made smaller and more refined appearing. A splint on the outside of the nose is worn for 1 week. Sometimes there will be bruising under the eyes. Most patients will take one to two weeks to recover. Full recovery takes several months and subtle improvements can take place over a year.
Cheek implants can be placed through incisions in the mouth to give the cheeks a more dramatic and feminine appearance. Alternatively, temporary fillers can be used to achieve a similar result without surgery.
Tracheal cartilage shave
A prominent “Adam’s Apple” is a very masculine feature. The good news is that the cartilage that causes this appearance can be altered and reduced via a “tracheal shave”. A small incision placed over the cartilage is utilized to approach the cartilage. The procedure is failrly short and recovery is quick. The procedure does not typically alter the voice in any significant way.
Female to male procedures
The operation to masculinize the chest is called Subcutaneous Mastectomy because most of the breast tissue is removed from beneath the skin. The nipple/areola complex is left intact, although usually smaller.
