United Patients

There are changes to the US Anesthesia Partners-Washington Contract with United.

Please be aware that US Anesthesia Partners (USAP-WA) is no longer in-network with United. USAP-WA provides anesthesia services for our ASC. This impacts UnitedHealthcare commercial products, but does not affect UnitedHealthcare Medicare Advantage Plan patients. Please note that USAP-WA currently participates with all other major insurance plans in Washington.

Kaiser Patients

There are changes to the US Anesthesia Partners-Washington Contract with Kaiser.

The Polyclinic uses US Anesthesia Partners (USAP) to provide anesthesia services at our Surgery Center. The Polyclinic understands that USAP is out-of-network with Kaiser as of March 1, 2021, so please ask your insurer if or how this might affect your coverage.

Our Process. Your Options.

We understand that figuring out how to finance your procedure can be stressful. To help you navigate this we created this resource to help explain your options. We work with a variety of healthcare lenders, many of whom offer instant approval. We’d be happy to provide you with additional information on this option so that you can get started right away on your journey towards personal transformation.

We will discuss the exact costs for your procedure during your consultation. You will walk away knowing exactly what all costs will be including all related fees for anesthesia, facility, overnight stays, and any additional fees that may be incurred during recovery.

Clarity is empowering and with that in mind, we are always fully transparent. This will enable you to properly prepare for your treatment. We will answer all of your questions and will provide additional consumer information on our lenders. We accept all major credit cards, as well as cash, checks, and money orders.

Your Health Plan: Coverage, Co-pay, Provider Networks.

We encourage all patients to check with your health insurance company before making an appointment to learn about your insurance coverage, co-pay requirements and provider networks. (Be sure to note which physicians, clinics and hospitals are included in your provider network.) Insurance can change throughout the year. Please contact us to see if we are a  participating provider with your insurance to ensure access and coverage.

CosmetAssure. Insurance coverage for Cosmetic Patients.

At the Polyclinic Plastic Surgery Center, all of our cosmetic patients have CosmetAssure coverage for their procedure. Coverage varies by procedure and will be discussed during your consultation. Every surgery, including elective cosmetic surgery, carries the risk of a complication. CosmetAssure was created in 2003 by board-certified plastic surgeons because most major health insurance policies excluded payment for treatment of a complication from an elective cosmetic surgery.

As the number of elective cosmetic procedures increase, so does the number of medical complications arising from these procedures. CosmetAssure is an affordable solution to the lack of insurance coverage for medical complications that can occur during or after surgery. CosmetAssure provides peace of mind to both board certified surgeons and patients in the unlikely event a complication were to occur. Surgeons can execute the best medical care knowing the patient will not suffer a financial loss due to post surgical medical bills.

Coverage Requirements

CosmetAssure covers patients during the 45 days immediately following their surgery with a board certified surgeon. In the event of a complication, treatment at an accredited facility will be covered up to the maximum limits of the policy.
